By completing this agreement, you authorize the provider to investigate and inspect your physical location, IF required to establish the account.  If you are not currently an EZdesk / LoanProz member, you must become one.  The charge for account set-up is $39 plus $8.95 per month.  IF a physical inspection is required of your business location, a fee of $79 will be charged to your credit card at the time the inspection is required by us or providers.
